• Rollovers cannot be processed through NetBenefits. Please contact your plan administrator.
• Prospectuses will be mailed upon request.
• For sponsor-approved loan and withdrawal requests, completed forms should be returned to the employer in order for processing to continue.
• If the amount available for loans or withdrawals becomes less than the amount you requested on the processing date, the transaction will be cancelled.
• Federal law requires that you receive a Special Tax Notice Regarding Plan Payments from your Plan Administrator generally no less than 30 days nor more than 90 days before your distribution date.
